OUR PURPOSE 


Our goal is to provide you with the finest aesthetic medical services possible. What makes this office unique is our commitment to excellence and our dedication to patient care and safety. The purpose of this document is to  introduce you to Dr. Johnson and the Capital Aesthetic + Laser Center practice. If you should have any questions, please do not hesitate to call. 


OFFICE LOCATION 


Our office is located in Foggy Bottom in the heart of Washington, DC. Parking is available in the parking garage within the building or at metered street parking.

Office Hours 

Our office is open from 10:00 a.m. to 5:00 p.m. Monday through Friday and Saturdays by appointment only.

Our office is a state of the art medical facility with the latest advances in laser and aesthetic technology. Our award winning office is luxurious and refined while being private and discreet. 


Confidentiality 

Our office is fully committed to compliance with HIPAA & PHI guidelines by: 

● Providing appropriate security for our patient records. 

● Protecting the privacy of our patient’s medical information. 

● Providing our patients with proper access to their medical records. 

● Appropriately maintaining our patient information and billing processes in compliance with national standards.

Smoking Policy 

Smoking is not permitted in or around our building at any time. 


Children 

We love children; however, in consideration of other patients and to allow your visits to be as focused as possible, we kindly request that children not accompany our patients to their scheduled appointments. 


Appointments In order to provide the best possible service to our patients, we request that you arrive in plenty of time for your appointment. 

We suggest that you arrive 15 minutes early for a “New” patient visit (to allow time to complete registration paperwork), and 10 minutes early for all other visits. 


Patients who arrive 15 or more minutes late may be asked to reschedule if his or her treatment cannot performed in the amount of time remaining in his or her appointment slot.  If patient is rescheduled then a $50 cancellation fee will be incurred. 


FINANCIAL POLICY 

Fees 

All fees for procedures are payable cash or credit card. We do accept VISA, MasterCard, AMEX and CareCredit for your convenience.  


Insurance 

Our office will not bill insurance for any cosmetic procedure performed. The patient is responsible for all professional services performed. Procedure fees will be discussed openly with you before scheduling procedure. 


Initial Consultation Fee 

For a first-time cosmetic procedure consultation with our providers, the fee is $100 and is collected when the appointment is scheduled. If patients schedule procedures within 30 days of their consultation, the entire $100 consultation charge will be applied toward the procedure fee. 


Patients will also be required to place a scheduling deposit of $50 when making appointments with our providers. In the event that you cannot make your scheduled appointment, please contact us to cancel within two (2) business days prior to your scheduled appointment in order to receive a refund of your deposit. 


Credit Card Authorization 

For the convenience of our patients, product purchases and payment for services may be facilitated over the telephone. Charges for these transactions will be made to the credit card provided by the patient. You will be advised of the amount charged at the time of your purchase; however, please understand that there may be additional fees for packaging, handling, shipping or postage. 


Cancellation & No-Show Policy 

Availability for appointments is very competitive. In the event that you cannot make your scheduled appointment, please contact us to cancel within two (2) business days prior to your scheduled appointment. Any less notice will result in a loss of your scheduling deposit. Additionally, patients who no-show appointments will forfeit the deposit or 1 treatment (if scheduled for a pre-paid series treatment).


Refund Policy 

We do not offer refunds on any pre-paid treatments or series of treatments. Patients wishing to cancel future or to discontinue treatments will be entitled to a practice credit equal to the prorated value of remaining treatments. Additionally, due to the nature of the products, we do not offer refunds or returns of any CA+LC SKIN products.
